Physical Assessment Clinical Manifestations
The goals of nursing assessment are the establishment of baseline data and the early detection of and prevention of increased ICP, systemic hypotension, hypoxia, or hypercapnia.
Because it is estimated that 5% to 20% of clients with head trauma have associated cervical spinal cord injuries, all clients with head trauma are treated as though they have spinal cord injury until radiographic studies prove otherwise.
